You must fill out the google form before care can be started.  If your child utilizes respite money through the Family Support program, a subcontract must be signed by all parties before starting care.  

The information on this form must be kept current.  If there is any change, the parents do hereby agree that they shall notify the provider immediately and fill out a new form.  

It is the parent’s responsibility to provide such things as diapers/pull-ups, change of clothes, powders/ointments, medication, etc.
